Demonstrations and "Free Offer" Forms :

AOA History form (in Microsoft Word©.doc format) This is the form referred to in the AOA Journal article about detecting and prescribing for more of a patient's visual requirements. The form is editable in order for you to alter policy and other location information.

Most Recent NETwork main files: Right Click to download an updated NETwork main database file for our past clients: NET.MDB This file should be saved to the c: \ NETWORK folder, overwriting the existing file of the same name. The following two files must also be saved, but to the C:\netdata folder. Once downloaded, you can drag and drop them into the netdata folder and let them overwrite the existing file. CONNECT1.MDB and CONNECT1.LDB The two updated Connect1 files are required for this update's referral recording system to work. Note: Many other files are required to make NETwork function. If you are not a licensed user (past client) you will not be able to make the system work.
